Hard water can influence what remains on skin after washing

Danby and colleagues studied 80 participants, including healthy controls and people with atopic dermatitis, and compared washing with water of different hardness levels. Sites washed with hard water had significantly increased deposition of sodium lauryl sulfate. The deposited surfactant was associated with increased transepidermal water loss and irritation, particularly in participants with atopic dermatitis and certain FLG mutations. This matters for facial cleansing because a cleanser is only one part of the washing system. The water used to dilute, spread and rinse the cleanser can influence the final skin environment.

Hard-water effects are not the same as cleanser irritation

Research on skin cleansing more broadly shows that surfactants can interact with proteins and lipids in the stratum corneum. A systematic review and meta-analysis identified 16 studies examining water hardness, atopic eczema and skin-barrier outcomes. Pooled observational data found higher odds of atopic eczema in children exposed to harder water, but the certainty of that association was rated very low.
